Quote and Enrollment Process for Benefits Connection
It's easy to get a quote! For groups with 1-49 employees the Benefits Connection can provide a variety of plans and coverage types.
Below we have outlined the quoting and enrollment process so that you will know what is needed and the overall process. How To Get A Quote
- Print the 'Quote Package'
- Employer will complete the Census and Group Form
- Each full-time & eligible employee is required to complete a 'Wellness Form'
- When all forms are completed, they can be mailed (410 Delaware Ave Wilmington, DE 19801), emailed (allen@alleninsurance.com) or faxed (302-654-8836).
- Upon receipt of the forms they will be provided to the insurance companies for rating.
- Normally, within 3-4 days the quote is received and we will call you to discuss the premium and will provide a formal quote to you for your review.
The Enrollment Process
Completed enrollment forms are required by the 15th of the month for coverage to be effective the first of the following month. The following forms are required:
- Signed insurance company application
- Completed employee application(s)
- Participation Form
- First month premium
- Chamber membership application for appropriate chamber (if not a current member)
- Sole Proprietors must include Schedule C of their tax return
- Corporations must include their most recent UC8 Form (Unemployment Compensation Quarterly Report)
