Choice PPO is the only plan that shows orthodontic coverage. It carries a $1,000 lifetime maximum for orthodontics.
Choice Active PPO offers a $2,500 in-network annual maximum. Out-of-network maximum is $1,500.
Choice is a broader PPO network. Select is a tighter network that often delivers stronger in-network benefits. Your advisor can show provider access for both.
